The Crawfordsville Mobile Integrated Health-Community Paramedicine Program's team will provide a brief overview of their successes in starting a chronic disease management program in 2017, and how they've built robust programs and partnerships over the last six years to extend care through paramedics, nurses, and social work to address patients healthcare, social needs, and assist patients 60 years and older to age in their homes.
